Caregiver Navigation Program: Support When You Need It Most
Caring for a loved one can be rewarding—but it can also be overwhelming. Philadelphia Corporation for Aging’s Caregiver Navigation Program helps caregivers connect to the right resources and support, so you don’t have to figure everything out on your own. It’s designed to strengthen caregiver well-being while helping you better support the person in your care.
This free program offers information and referrals tailored to your situation, along with personalized care consultation that centers your needs and preferences. You can also access trainings, workshops, and educational resources to help you navigate caregiving with more confidence—whether you’re caring for an adult age 60+ (or a person with dementia at any age), or you’re an older relative (55+) raising children as the primary caregiver in your home. The care receiver must live in Philadelphia County.
