Joint Advisory Council Meeting

How are Social Determinants of Health Impacting Older Adults?

Register today and join us at PCA on Thursday, December 5, 2024, from 10:00 a.m. – 1:00 p.m. for our Joint Advisory Council Meeting. This meeting will feature presentations on:

  • Social Determinants of Health and Older Adults
  • Social Engagement Programs
  • SNAP Enrollment
  • PCA Program Updates
