Help Yourself to Health: A Chronic Disease Self-Management Program

Dates: Wednesday – Sept. 11, 18, 25, Oct. 2, 9, and 16
Time: 12:30 p.m. – 3:00 p.m.

Help Yourself to Health is a chronic disease self-management program that empowers older adults to effectively manage chronic health conditions, including but not limited to diabetes, heart disease, arthritis, and high blood pressure. This in-depth, six-week program meets weekly for 2.5 hours, during which participants learn to take charge and manage their conditions and any associated symptoms, such as pain, depression, and physical limitations. This program is offered at no cost to those 60 and older who live in Philadelphia.

Program topics include:

  • Combating fatigue and getting a good night sleep
  • Address symptoms such as pain, shortness of breath, and emotional difficulties
  • Healthy eating
  • Physical activity
  • Communication skills
  • Falls prevention
  • Relaxation and breathing techniques
  • Action planning, decision making, and problem solving
